As a member of our family, you are entitled to the convenience of all our in-house services, as well as complete access to first class medical care. How to become a new Patient

  • Choosing your doctor:
    Your primary care physician oversees all aspects of your health care including: physical exams, preventive care and referrals to specialists. Before calling and scheduling an appointment. Take some time to read through the biographies of our highly skilled medical physicians. Profiles of all our physicians are available in the Meet Our Doctors section.

    Family Practice, Internal Medicine and Pediatric, Sports Medicine physicians are available as primary care physicians.

  • Making an appointment:
    Once you have selected your physician of choice, you are now ready to make an appointment. We offer a variety of options to make your first appointment. The options are listed below: Torrance Office: (310) 378-2234

    You may also request, cancel or confirm an appointment online or by phone at least 24 hours in advance.

  • Fill out the required "New Patient" forms. We offer a variety of options to access our forms. The options are listed below:
    • New Patient Forms are located through our secure online patient portal. Click here to access our portal.
    • Forms are also located on our website, you may print out your form and ether fax or submit in person. Click here to access the forms page.
    • New patient forms are also available at each of our offices. You may fill out a form at the office. Please be sure to arrive early.

    We recommend that you fill out these forms and return them to us before your appointment.

  • Transferring Medical Records
    If you are a new patient, you will need to complete a medical records release form after your initial visit. Learn more about medical records.

    Prior to your appointment, you will be contacted by telephone to verify your appointment. Thank you for your consideration and we look forward to establishing a long relationship with you and your family.

Care Available When You Need It:

South Bay Family Medical Group's Urgent Care Center provides convenient medical care for illnesses requiring non-emergency, immediate care. We are available to care for you and your family on a walk-in basis. Our Urgent Care Center does not require an appointment and is available to everyone,14 years of age and older, even if you are not currently a South Bay Family Medical Group patient. On-site Services:

Laboratory, bone density, x-rays, EKGs and more are readily available to all patients. Learn more about on-site services.

Prescription Refills:

Allow 4-7 days for prescription refills. Call the pharmacy where you filled the prescription; the pharmacy will send a request to your doctor for a new authorization if you do not have any remaining refills. If you have not seen a local South Bay Family Medical Group doctor yet and need a refill, call to make an appointment and bring all your medications with you.
